You will work closely with patients, families, therapists, and referral sources to ensure high-quality care. This includes handling case assignments, drafting service plans, reviewing case progress, and determining case closure. You will also communicate patient needs to various care team members and follow up accordingly. Furthermore, you will manage discharge plans upon completion of treatment and attend clinical and administrative meetings for inter-departmental sharing of information.
