Job Description
The Case Manager provides group therapy and related documentation services, along with additional ancillary services. The Case Manager is a highly qualified individual with experience regarding the nature of alcoholism and drug addiction. The Case Manager will need to be familiar with 12-Step Recovery programs and have knowledge of documenting group notes in an EMR system. Must have the ability to work as a team member and work collaboratively with the Primary Counselor and Clinical Director in providing quality treatment services and client care.
Your primary responsibilities will include coordinating and providing care that is safe, timely, effective, efficient, equitable, and patient-centered. You will work closely with patients, families, therapists, and referral sources to ensure high-quality care. This includes handling case assignments, drafting service plans, reviewing case progress, and determining case closure. You will also communicate patient needs to various care team members and follow up accordingly. Additionally, you will manage discharge plans upon completion of treatment and attend clinical and administrative meetings for inter-departmental sharing of information.
