The Role
We are seeking a high-energy, clinically skilled Licensed Social Worker (LSW) to serve as a Behavioral Health Care Manager (BHCM). In this role, you are the "engine" of the Collaborative Care team, working alongside Primary Care Providers (PCPs) and our Psychiatric Consultants to manage a caseload of patients toward clinical remission.
Key Responsibilities
- Case Management: Oversee a registry-based caseload of adult patients, systematically tracking progress using validated tools like the PHQ-9, GAD-7.
- Clinical Interventions: Deliver brief, evidence-based therapeutic interventions including Behavioral Activation, Motivational Interviewing.
- Collaborative Review: Participate in weekly systematic case reviews with the Zenora Psychiatric Consultant to refine treatment plans and medication recommendations.
- Care Coordination: Facilitate "warm hand-offs" from PCPs and communicate intake reports and diagnostic impressions back to the primary care team.
- Outcome Tracking: Manage the patient registry to ensure "Treatment to Target" goals are met, typically aiming for symptom remission within 3 to 6 months.
Qualifications
- Licensure: Active New Jersey Licensed Social Worker (LSW) in good standing.
- Education: Master’s Degree in Social Work (MSW) from an accredited institution.
- Experience: Fresher/Experience in medical social work, primary care integration, or outpatient mental health is preferred.
- Clinical Skills: Familiarity with short-term, evidence-based counseling modalities (CBT, MI).
- Tech-Savvy: Comfortable using digital registries, AI-driven intake platforms, and EHR systems.
