Licensed Clinical Social Worker (LCSW) must be Licensed in New Jersey.

Zenora Health is seeking a Clinical Supervisor & Counselor to lead the charge in operationalizing high-impact, integrated care models. This is not just a management role; it is a strategic leadership position designed to bridge the gap between behavioral health and primary care. We are looking for a visionary with deep expertise in the Collaborative Care Model (CoCM), Behavioral Healthcare Integration (BHI) and the new Advanced Primary Care Management (APCM) framework to ensure our clinical operations are seamless, compliant, and patient-centered.

Key Responsibilities

· Operational Leadership: Drive the implementation and scaling of integrated behavioral health services across primary care sites.

· Clinical Supervision: (For LCSW candidates) Provide formal clinical supervision to on-site LSWs, ensuring high standards of care and regulatory compliance.

· Workflow Integration: Design and embed behavioral health protocols into existing EHR workflows that enhance—rather than disrupt—primary care provider (PCP) productivity.

· Financial & Regulatory Oversight: Manage CMS billing thresholds and ensure strict audit readiness for APCM G-codes and CoCM billing.

· Quality & Data Management: Oversee MIPS Value Pathways (MVP), track patient registries, and ensure data integrity for quality reporting.

· Physician Engagement: Act as a change agent to gain buy-in from PCPs, providing the training and support necessary for a successful integrated culture.

Required Credentials:

· LCSW (Licensed Clinical Social Worker): Highly Preferred. The ability to provide clinical supervision to our LSW team is a strategic priority for this role.

· OR Specialized RN: Registered Nurse with significant psychiatric experience and a proven track record in clinic management.

Licensure: Must hold a valid New Jersey State License.

Bilingual: Fluency in a second language will be an addition to support our diverse patient population.

Technical Competencies:

· CoCM/APCM/BHI Mastery: Must have a practical knowledge of Collaborative Care Model mechanics, Behavioral Health Integration (BHI) and the new APCM G-code requirements.

· Systems Thinker: Proven experience navigating EHR integrations and optimizing clinical workflows.

· Quality Maven: Hands-on experience with MIPS, registry management, and value-based care metrics.

· Influencer: Exceptional communication skills with the ability to lead change management among medical staff.
