Aftercare Coordinator

About Youth Opportunity & Osceola Youth Center
Youth Opportunity is a nationally recognized leader in residential and behavioral health treatment for at‑risk youth. Osceola Youth Center in Evart, Michigan is a trauma‑informed Youth Center serving boys ages 10 to 17 requiring behavioral stabilization. The program merges clinical therapy, education, life skills training, and expressive recreation to support emotional growth in a structured setting.
Youth typically remain in care for 6 to 9 months, engaging in individual, family, and group therapy; restorative justice; victim‑impact awareness; and pre‑vocational preparation. Programming in small group units encourages family partnership and successful return to the community.
About the Role
The Aftercare Coordinator provides ongoing support, resources, and follow-up services to youth after they have completed the program or transitioned to community-based care. This role ensures a smooth transition, promotes long-term success, and helps clients maintain stability through continued engagement, case management, and community connection.

Key Responsibilities
Develop, implement, and manage individualized aftercare plans tailored to each youth's needs and goals.
Conduct regular follow-up calls, meetings, or home visits to monitor progress and provide ongoing support.
Maintain accurate, timely documentation of client interactions and outcomes in the case management system.
Connect youth with community resources such as housing, employment, education, mental health, and substance use recovery programs.
Collaborate with program staff, case managers, and community partners to ensure continuity of care.
Track and evaluate aftercare outcomes to identify trends and areas for improvement.
Serve as an advocate for youth navigating post-program challenges, ensuring access to needed resources.
Provide crisis intervention and referrals when necessary.
Support program events, outreach, and alumni engagement efforts as needed.
Participate in group activities and maintain a high standard of role model behavior for youth.
Participate in training, development, and site routines as required.
Report incidents, unusual occurrences, and any potential boundary or safety concerns per guidelines.
Perform other duties as assigned.
Qualifications
Education: Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or related field required.
Experience: 1–3 years of experience in case management, counseling, social services, or recovery support. Experience working with youth with emotional or behavioral challenges preferred.
Skills:
- Strong understanding of group leadership, dynamics, and experiential learning.
- Excellent written and verbal communication skills.
- Empathy and cultural sensitivity with diverse populations.
- Ability to manage multiple priorities and maintain accurate records.
- Knowledge of community resources and social service systems.
- Proficiency in documentation and database management.
Crisis intervention and conflict resolution abilities.
Other Requirements:
- Valid state driver's license and ability to meet driving record requirements.
- Must meet state criminal background check and licensing requirements.
- Ability to occasionally work irregular hours as needed.
Physical Demands
Must be in good physical health and emotionally stable
Able to actively engage with youth, participate in physical interventions, and transport youth or staff as needed
Must be free of communicable diseases as defined by the state
Must pass evaluation of physical techniques and certification exams
