Youth Advocate Programs, Inc. (YAP) is a national nonprofit that provides community-based alternatives to youth incarceration and residential placement. Founded in 1975 and headquartered in Harrisburg, PA, YAP partners with youth justice, child welfare, behavioral health, and education systems to deliver individualized, wraparound services that keep youth safely in their homes and communities.

YAP’s model focuses on building strong relationships with youth and families, connecting them to local resources, and supporting positive life outcomes through strength-based, culturally responsive approaches. Advocates work directly in the community to help participants develop skills, meet goals, and strengthen family and community connections.

Status: Part- Time Hourly                                              

FLSA Classification: Non-Exempt

Hourly Rate: $20.00 

Summary of Position:

Family Support Workers partner with children and families to help them achieve stability, strengthen relationships, and reach their individualized goals. Using YAP's Wraparound model, Family Support Workers provide direct support, advocacy, and coordination of services designed to help families build on their strengths and overcome challenges. Services are delivered in homes and community settings, allowing staff to make a meaningful impact where families live, learn, and grow.

  • Provide direct services and supports to children and families as identified in Individualized Service Plans (ISPs).
  • Coordinate services and interventions while maintaining fidelity to YAP's Wraparound advocacy model.
  • Implement strategies and interventions identified through the ISP process.
  • Support families in achieving goals identified through individualized service planning.
  • Maintain a caseload of no more than six (6) families to ensure intensive, individualized support.
  • Document services and activities in accordance with program requirements.
  • Work collaboratively with Supervisors, Program Directors, families, and other service providers to support successful outcomes.

Qualifications/Requirements:

  • Bachelor’s degree in Social Work, Psychology, Human and Child Development, Counseling, Sociology, or a related field required.
  • Minimum of one (1) year of experience working with children, youth, and/or families in a human services, behavioral health, education, or community-based setting.
  • Knowledge of strengths-based, family-centered, and trauma-informed practices preferred.
  • Ability to engage and build positive relationships with youth, families, and community partners
  • Strong organizational, time management, and problem-solving skills with the ability to manage multiple priorities.
  • Excellent verbal and written communication skills.
  • Proficient in Microsoft Office; experience using an electronic health record (EHR) system is preferred.
  • Bilingual (Spanish/English) candidates are encouraged to apply.
  • Must possess a valid driver's license, reliable transportation, and current auto insurance coverage.
  • Ability to work a flexible schedule, including evenings and weekends, as needed to meet family needs.
