Key Responsibilities:
- Provide comprehensive primary care services across all stages of life in an outpatient setting.
- Diagnose, treat, and manage acute and chronic health conditions.
- Deliver preventive care, wellness visits, health education, and chronic disease management.
- Develop long-term relationships with patients and families through continuity of care.
- Collaborate closely with physicians, advanced practice providers, nursing staff, and other healthcare professionals.
- Maintain accurate clinical documentation using electronic medical records.
- Participate in quality improvement initiatives and promote evidence-based patient care.
Qualifications:
- Licensed Family Nurse Practitioner (FNP).
- Nurse must have an RN license or be eligible for an RN license in the state of NY
- Experience in primary care, family medicine, internal medicine, or community health preferred.
- Strong commitment to patient-centered, relationship-based care.
- Excellent communication, collaboration, and clinical decision-making skills.
