We have a new opportunity for an Intake Coordinator in our Home Care team. This is a full time, 40 hours a week position and is located in York, Maine.
The Home Care Intake Coordinator is responsible for intake of referrals and physician orders, development of exceptional relationships with referral sources, coordination of ordered services for: Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Work and Home Health Aide. Promotes excellence in clinical practice.
- Develops and maintains relationships with referral sources.
- Receives and completes home care referrals and physician orders.
- Evaluates referred patient needs and coordinates admissions with staff availability, ensuring effective coverage.
- Partner with other community providers regarding continuity and coordination of patient care.
- Communicates frequently with team members for the purpose of ongoing assessment of the patient's need for continued intervention and response to the plan of care, ensuring that all care is provided in accord with physician orders and in compliance with home care regulations.
- Identifies and communicates quality of care issues to Director of Home Care for quality improvement activities.
Makes recommendations regarding educational needs of the Director of Home Care
Liaison:
- Partner with York Hospital Care Management Team and attend morning case conference
- Attend community monthly networking meetings (i.e Healthcare Network Group)
- Visit local family practice offices, hospitals, and skilled nursing facilities to share York Hospital Home Care information and services.
- Participate in various Health Care fairs to represent York Hospital Home Care.
Education/Licensure/Certifications/Qualifications:
- A registered nurse or licensed practical nurse currently licensed in Maine, with three to five years of progressive nursing experience. One year of home healthcare experience preferred.
- Demonstrated ability to interpret information related to patient care needs, understanding the range of treatment required for all diagnostic groups.
- Demonstrated expertise in home care regulations and coding.
- Ability to manage multiple priorities while maintaining exceptional relationships with patients, team, and referral sources.
- Current valid license to operate a motor vehicle. Car available for work.
