Position Summary

The Certified Nursing Assistant II (CNA II) provides direct patient care under the supervision of a Registered Nurse or Licensed Practical Nurse. In addition to performing all duties of a Certified Nursing Assistant I, the CNA II is authorized by the North Carolina Board of Nursing to perform advanced skills within their approved scope of practice after successful completion of an approved CNA II program and competency validation. The CNA II assists in promoting patient safety, comfort, independence, and recovery while supporting the interdisciplinary treatment team.

Essential Duties and Responsibilities

Direct Patient Care

  • Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, eating, and mobility.
  • Assist with ambulation, transfers, and positioning using proper body mechanics.
  • Monitor and document food and fluid intake and output.
  • Assist with range-of-motion exercises and restorative care.
  • Observe patients for changes in condition and promptly report concerns to licensed nursing staff.

CNA II Clinical Skills (per NC Board of Nursing and facility policy)

May perform approved CNA II skills after competency validation, including:

  • Measure and record vital signs.
  • Perform finger-stick blood glucose testing.
  • Collect routine urine, stool, and sputum specimens.
  • Perform sterile and non-sterile dressing changes as delegated.
  • Catheter care and selected urinary catheter procedures within scope.
  • Tracheostomy care and suctioning as delegated.
  • Ostomy care.
  • Enteral feeding assistance (G-tube/NG tube) as permitted.
  • Oxygen therapy support.
  • Pulse oximetry monitoring.
  • Peripheral IV maintenance activities permitted under NC regulations (excluding initiation unless specifically authorized).
  • Other delegated CNA II skills consistent with North Carolina regulations and facility policies.

Behavioral Health Support

  • Maintain a therapeutic, trauma-informed, and person-centered environment.
  • Observe and document patient behaviors, mood, and participation in treatment.
  • Assist with patient safety checks and environmental rounds.
  • Support de-escalation efforts under clinical direction.
  • Encourage participation in treatment activities and daily routines.

Documentation

  • Accurately document care provided in the electronic health record (EHR).
  • Complete required flow sheets, vital sign documentation, intake/output records, and incident reports.
  • Report abnormal findings immediately to licensed nursing staff.

Infection Prevention & Safety

  • Follow infection control procedures and standard precautions.
  • Maintain a clean, safe, and organized patient environment.
  • Ensure proper handling of equipment and supplies.
  • Participate in emergency response procedures and safety drills.

Team Collaboration

  • Communicate effectively with physicians, nurses, therapists, family and other interdisciplinary team members.
  • Participate in shift reports and care conferences as requested.
  • Maintain patient confidentiality in accordance with HIPAA.

Minimum Qualifications

  • High school diploma or GED.
  • Current North Carolina Nurse Aide I certification in good standing.
  • Current North Carolina Nurse Aide II certification in good standing.
  • Current BLS/CPR certification.
  • Ability to successfully complete facility orientation and competency validation.
  • Ability to pass criminal background and drug screening.

Preferred Qualifications

  • One year of CNA II experience.
  • Experience in behavioral health, crisis stabilization, psychiatric, residential, or long-term care settings.
  • Experience using electronic health records (EHR) and/or shift notes.

Knowledge, Skills, and Abilities

  • Knowledge of basic nursing principles and patient care.
  • Strong observation and communication skills.
  • Ability to recognize changes in patient condition.
  • Excellent interpersonal and customer service skills.
  • Ability to maintain professional boundaries.
  • Ability to work independently and as part of an interdisciplinary team.
  • Knowledge of trauma-informed care and crisis intervention principles preferred.

Physical Requirements

  • Frequently lift up to 50 pounds with assistance when appropriate.
  • Stand, walk, bend, stoop, and reach for extended periods.
  • Assist with patient transfers and repositioning.
  • Manual dexterity sufficient to perform patient care procedures.

Pay: $15.00 - $20.00 per hour
