Role Description
We are seeking a full-time Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in Hartford, CT, for an on-site role. The NP/PA will deliver comprehensive patient care, including conducting assessments, diagnosing conditions, developing treatment plans, prescribing medications, and providing ongoing management and follow-up. This role involves working collaboratively with healthcare teams to ensure optimal patient outcomes and education for patients and their families in alignment with our commitment to community health.
Qualifications
- Strong clinical assessment and diagnostic skills
- Proficiency in creating and managing treatment plans and prescribing medications
- Excellent communication and patient education skills
- Ability to collaborate with interdisciplinary healthcare teams and prioritize patient care
- Valid state licensure as a Nurse Practitioner (NP) or Physician Assistant (PA)
- Certification in family medicine, internal medicine, or a relevant specialty
- Experience in outpatient care or primary care is a plus
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications
- Commitment to patient-centered care and serving diverse populations
