Yamhill County has two regular full-time positions for Community Health Workers (CHW) to provide health navigation and support services to students and families referred by participating schools. In this position, you will be responsible for providing care coordination, case management, and connections to physical health, behavioral health, dental health, social services, and other community resources. CHWs will meet with students and families in schools, family homes, community locations, or other settings preferred by the family and provide ongoing follow-up to help them access needed services. Duties will also include responding promptly to referrals, collaborating regularly with school staff and community partners, maintaining accurate and confidential records, reporting on program activities and outcomes, and identifying opportunities to continually improve services for students and families.

The Qualifications:

  • Bachelor’s degree in a human services related field.

OR

  • A combination of at least one year’s work experience and two years’ post secondary education.

OR

  • A high school diploma and three years’ work experience in human services.

Any satisfactory equivalent combination of education, training, and/or experience relevant to the position.


Must be eligible to receive and maintain certification as a Community Health Worker.


The Candidate:

As a successful candidate for this position, you should be able to demonstrate excellent communication, organization and relationship-building skills, along with the ability to work effectively both independently and as part of a team. You should be comfortable engaging with students and families in schools, homes and community settings and collaborating with school staff and community partners to connect families with needed resources. It is also expected that you will maintain accurate and confidential records, follow through consistently on referrals and provide services in a courteous, respectful and culturally responsive manner.
