Overview

Join our dedicated team as a Community Health Worker and become a vital link in promoting health, wellness, and equitable access to care within underserved communities. The Traditional Health Care Worker (THW)/Community Health Worker (CHW) serves as a trusted liaison between WVP Health Authority, healthcare providers, and the communities they serve. The THW promotes health equity by providing culturally responsive outreach, education, care coordination, advocacy, and resource navigation. This position helps individuals and families overcome barriers to healthcare, improve health outcomes, and connect with community resources that support their overall well-being.

Duties

  • Build trusting relationships with patients, families, and community members.
  • Conduct outreach to individuals who may benefit from healthcare and social support services.
  • Assist patients in navigating healthcare systems, scheduling appointments, and understanding treatment plans.
  • Connect patients with community resources, including housing, food assistance, transportation, behavioral health services, financial assistance, and other social services.
  • Provide culturally appropriate health education and promote disease prevention and healthy lifestyle choices.
  • Support patients in developing self-management skills for chronic conditions.
  • Collaborate with healthcare providers, care coordinators, behavioral health teams, and community partners to ensure coordinated care.
  • Advocate for patients by identifying barriers to care and helping develop solutions.
  • Maintain accurate documentation of patient interactions and services provided in accordance with organizational policies and confidentiality requirements.
  • Participate in interdisciplinary team meetings, case conferences, and community outreach events.
  • Maintain knowledge of local resources and available community services.
  • Comply with all HIPAA regulations, Oregon Health Authority THW standards, and WVP Health Authority policies and procedures.
  • Participate in ongoing training and professional development to maintain certification and enhance skills.

Requirements

  • Current Oregon Traditional Health Worker (THW) certification.
  • High school diploma or GED.
  • Two years’ experience working with diverse populations and underserved communities.
  • Knowledge of community resources and social service systems.
  • Strong communication, interpersonal, and advocacy skills.
  • Ability to work independently and collaboratively within multidisciplinary teams.
  • Basic computer skills, including electronic health records and Microsoft Office applications.
  • Valid driver's license, reliable transportation, and ability to travel within the service area as needed.
  • Two years sobriety from any substance

Working Conditions

This position includes work in clinical settings, community locations, and patients' homes. Occasional evening or weekend hours may be required to support community outreach activities. Travel throughout the service area may be necessary.
