This role combines expert clinical care with patient education, collaboration with interdisciplinary teams, and proactive coordination with physicians and specialists. The NP is responsible for assessing, diagnosing, and treating acute and chronic wounds, ensuring continuity of care, and helping patients achieve optimal comfort, recovery, and quality of life.
Responsibilities:
- Conduct comprehensive wound assessments and documentation (location, size, depth, staging, exudate, odor, infection signs, pain level, etc.).
- Diagnose wound etiology and contributing factors (vascular, pressure, diabetic, traumatic, surgical, etc.).
- Perform debridement (sharp, enzymatic, or autolytic as appropriate).
- Apply appropriate topical agents, dressings, and advanced wound therapies (e.g., negative pressure wound therapy, compression wraps).
- Prescribe medications, antibiotics, and wound-care supplies within the scope of practice.
- Order and interpret diagnostic tests (e.g., wound cultures, imaging, labs) to guide clinical decisions.
- Monitor wound progression and modify care plans accordingly.
- Work collaboratively with physicians, RNs, LVNs, and other healthcare professionals to coordinate wound management plans.
- Communicate clearly with home health or hospice case managers to ensure continuity of care.
- Participate in interdisciplinary case conferences and quality review meetings.
- Provide consultative wound evaluations in facilities, homes, or clinics.
- Serve as clinical liaison between agency, hospitals, SNFs, and primary care providers.
- Educate patients and caregivers on wound-care techniques, prevention, nutrition, and hygiene.
- Develop educational materials and teach-back methods to reinforce understanding.
- Promote self-care and prevention of wound recurrence.
- Support end-of-life wound management for hospice patients, focusing on comfort and dignity.
- Complete detailed wound documentation and progress notes in the EMR system.
- Ensure all orders and care plans comply with Medicare, Medi-Cal, and payer regulations.
- Maintain accurate wound photo documentation per agency policy.
- Participate in chart audits and quality assurance initiatives.
- Uphold HIPAA and infection control standards at all times.
Qualifications
- Active and unrestricted California Nurse Practitioner (NP) License.
- Current NPI and DEA registration (if applicable).
- Certification in Wound Care preferred (e.g., CWCN, WOCN, CWS).
- Minimum 2 years of experience in wound management, home health, or post-acute care.
- Strong knowledge of wound-healing physiology, infection management, and dressing modalities.
- Excellent communication, assessment, and documentation skills.
