In this role, you will be responsible for:
- Administering medications and treatments as prescribed by healthcare providers
- Monitoring and documenting patient progress, vital signs, and other health-related information
- Assisting residents with daily living activities and personal care needs
- Collaborating with interdisciplinary teams to develop and implement individualized care plans
- Providing emotional support and education to residents and their families
- Ensuring a safe and sanitary environment by adhering to infection control protocols
- Active LPN license in the state of New York
- Minimum of 1 year of long-term care nursing experience
- Strong assessment and critical thinking skills
- Excellent communication and teamwork abilities
- Current BLS certification, with further certifications preferred
