Registered Nurse, RN - Case Management
40-hour position
WORK Inc. is seeking a Registered Nurse (RN) to join its nursing team to provide nurse case management and Medication Administration oversight to its community residential programs.
The ideal candidate will provide nursing support to residential staff with a high degree of autonomy and be able to independently identify individual’s needs and provide appropriate intervention. Supervision and support from Director of Health Services.
Nursing Responsibilities
- Attending all medically involved intake/discharge/team meetings.
- The oversight of all Behavior Modifying Medication Treatment Plans.
- Providing alternate and program specific trainings related to Health and well-being of the individuals.
- Visiting medically involved programs as needed.
- Leading the Health & Wellness Committee in collaboration with the Director of Health Services
- Identifying training required for staff on an ongoing basis.
- Tracking program specific trainings and schedule trainings with managers in collaboration with the Director of Health Services.
- Review medical progress notes/medical visits/ensure follow-up has occurred in a timely manner.
- Meet with the Director of Health Services on a weekly basis for supervision.
MAP – Medication Administration Program
- Quarterly Program MAP review (including feedback and follow-up).
- Assisting the Director of the Health Services with MAP training/MAP recertification training.
- Overseeing Quality Enhancement (QE) preparation for all MAP related reviews.

- Strong Knowledge of relevant regulations, compliance standards, and best practices in residential life.
Hours:
The position is 40hrs., five days per week with flexible hours and the only on-call responsibility will be PTO coverage. These are well established programs with excellent management retention and care. This position includes frequently going to residential homes and hospitals, rehab centers etc.

Salary : $75,000 - $80,000
