Licensed Practical Nurse (LPN) – PM Shift
Location: Woodlands Senior Park Assisted Living
Shift: PM Shift (1:00 PM – 9:00 PM)

Woodlands Senior Park Assisted Living is seeking a compassionate, dependable, and dedicated Licensed Practical Nurse (LPN) to join our PM shift team. The ideal candidate will have excellent communication skills, be able to work independently, and have a passion for providing compassionate care. Responsibilities:
- Administer medications and treatments according to physician orders.
- Assess residents and monitor changes in condition.
- Communicate with physicians, families, and the interdisciplinary care team.
- Supervise and support Resident Assistants/Caregivers.
- Complete accurate documentation in a timely manner.
- Respond appropriately to resident emergencies.
- Ensure compliance with state regulations and facility policies.
- Promote a safe, respectful, and welcoming environment for residents.
- Provide direct patient care, including taking vital signs, administering medications, and providing wound care.
- Assist with activities of daily living (ADLs).
- Provide education to patients and their families about health maintenance and disease prevention.
- Assist with rehabilitation and physical therapy.
- Provide geriatric nursing care to elderly patients.
Qualifications
Current, active LPN license in the State of Wisconsin.
CPR/BLS certification (or willingness to obtain).
Previous experience in assisted living, long-term care, or senior care preferred.
Strong communication, leadership, and organizational skills.
Compassionate, reliable, and team-oriented.
Ability to multitask and work independently.
