About The Role

Key Responsibilities
- Conduct monthly CCM/PCM outreach calls with enrolled patients to review symptoms, medications, and care plan progress
- Accurately document all encounters and time logs in eClinicalWorks (ECW)
- Coordinate care between specialists, PCP, and ancillary providers
- Assist with care plan creation and updates under provider supervision
- Identify and escalate urgent patient concerns to the clinical team
- Maintain compliance with CMS billing requirements for CCM/PCM time tracking
- Support patient enrollment and consent documentation
- Medical Assistant certification (CMA, RMA, or equivalent) preferred
- Minimum 2–3 years of experience in a clinical or care coordination role
- Proficiency in eClinicalWorks (ECW) — required, no exceptions
- Experience with CCM, PCM, or RPM programs strongly preferred
- Bilingual — English/Spanish required (spoken and written)
- Must reside in the Spring, TX or Katy, TX area
- Reliable home office setup with stable internet connection
- Strong communication, documentation, and time-management skills
- Ability to work independently with minimal supervision
- Pay: $17–$25/hour depending on experience
