Licensed Practical Nurse-3rd Shift Weekends

Duties

• Administer medications, including injections and IV infusions, following established protocols and safety standards
• Conduct comprehensive patient assessments, vital signs monitoring, and physical examinations to inform care plans
• Assist with complex procedures such as catheterization, tube feeding, wound care, and sterile techniques
• Document all patient interactions accurately using Electronic Health Record (EHR) systems while maintaining strict compliance with HIPAA and confidentiality policies
• Collaborate with interdisciplinary teams to develop and implement personalized care plans for residents with conditions such as dementia, Alzheimer's disease, or developmental disabilities
• Provide triage assessments for urgent needs and coordinate appropriate responses or referrals
• Support long-term care and memory care residents by delivering specialized services aligned with regulations and best practices

Requirements

• Valid state licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)
• Proven experience in long-term care facilities, skilled nursing facilities, or assisted living environments
• Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate documentation
• Knowledge of medical terminology, anatomy, physiology, and ICD coding (ICD-9/10) for precise medical records management
• Experience working with geriatric patients and individuals with chronic conditions like dementia or Alzheimer's disease
• Ability to perform physical examinations, vital signs assessment, specimen collection/processing, and basic life support (BLS) certification
• Strong communication skills for caregiver interaction and client communication in a compassionate manner
• Critical care experience or hospital/ER background is advantageous but not mandatory; enthusiasm for learning is valued

Pay: $31.22 - $40.00 per hour
