Job Summary

Responsibilities

• Assist patients and residents with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and transfer devices.
• Monitor and record vital signs accurately, including blood pressure, pulse, temperature, and respiration rates using electronic health record (EHR) systems like Epic or Cerner.
• Support medication administration under supervision, ensuring proper documentation in medical records while adhering to HIPAA regulations.
• Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care strategies and behavioral health interventions.
• Perform clinical tasks such as catheter care, phlebotomy procedures, aseptic techniques, and wound care in accordance with infection control protocols.
• Collaborate with multidisciplinary teams in acute care settings like Level I trauma centers or ICU units to deliver patient-centered care aligned with physiology knowledge and medical terminology standards.
• Assist with patient mobility using equipment such as Hoyer lifts or transfer belts; support post-acute care and outpatient services as needed.
• Maintain accurate medical documentation of patient service activities, vital signs, and changes in condition using EMR systems like eClinicalWorks or Home & Community Care software.
• Support patients across diverse populations including pediatrics, geriatrics, behavioral health cases, and individuals with disabilities or developmental disorders.

Qualifications

• Valid certification as a Nursing Assistant (CNA) from an accredited program; current state licensure required.
• Demonstrated experience working in healthcare environments such as hospitals (including ICU or trauma centers), nursing homes, assisted living facilities, or home care settings.
• Knowledge of medical terminology, anatomy, physiology, and aseptic techniques essential for delivering safe patient care.
• Experience with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks is highly preferred.
• Familiarity with specialized areas including Alzheimer’s/dementia care, hospice support, infectious disease protocols, and behavioral health is advantageous.
• Ability to perform basic life support (BLS) certification; additional certifications like Phlebotomy or DME experience are a plus.
• Strong communication skills coupled with a compassionate approach to caregiving; ability to work effectively within multidisciplinary teams.
