What are the responsibilities and job description for the Direct Support Professional for Supported Living with 2 to 1 Ratio Services position at WNC Direct Care, Inc?
Provide 24/7 in-home care for the Member served by the WNC Direct Care with (2) 12-hour shifts for available schedules Monday-Thursday or Friday-Sunday. Staff ratio is (2) staff to (1) member served.
Common daily tasks include medication administration, documenting data, log note writing, assist with daily skill development, ensure cleanliness of the home, attend medical and personal appointments with transportation scheduled by Guardian and/or Care Manager and QP supervisors.
SUPPORT SERVICES – In accordance with Member Support Plan, assist member in:
- completing self-care routines, as needed (may include, but is not limited to: bathing; dressing; nail care; hair care; oral hygiene; toileting; changing undergarments) and teaching skills that could promote a higher level of independence.
- completing self-help tasks, as needed (may include, but is not limited to laundry; meal preparation; sweeping; vacuuming; mopping; dusting; dishwashing; taking out trash; money management) and teach skills that could promote a higher level of independence.
- maintaining the best possible health by following prescribed plans of care (may include, but is not limited to: nutrition plans; health care plans; exercise regimes) and by promoting healthy choices; and,
- accessing the community (may include, but is not limited to shopping; participating in leisure activities; attending health-related appointments) by providing support and transportation (using a company and/or personal vehicle or public transportation)
- administering prescription, over the counter, and PRN medications, as ordered by a physician.
- as required, attend health related appointments with service recipient and communicate effectively on member behalf
- ensure service recipient’s home is clean, safe and free of maintenance concerns
- support service recipient in developing and maintaining relationships of his/her choice
- assist in the development of person-specific plans and implement all plans (e.g., Member Support Plan, Behavior Support Plan, Staff Instructions) designed to support member
- complete required documentation in a thorough manner and submit all required documentation to appropriate personnel in a timely manner within a 24-hour time period
- work assigned schedule in accordance with WNCDC Policy and Procedures
TEAM PARTICIPATION
- Work collaboratively with all WNC Direct Care personnel, promoting effective communication, conflict resolution, and interpersonal interactions appropriate to the workplace
- Attend Team Meetings and training, as required
- Complete other functions, as requested by supervisor and / or Administrator
- Perform all job functions in a manner that exemplifies WNC Direct Care’s mission and philosophy of service delivery and complies with all WNC Direct Care Policy and Procedures
LOCATION:
- Services are provided in residential homes leased/owned and paid for by the Member Served in Asheville, NC.
RATE OF PAY:
Monday-Thursday Day Shift – 12 hour @ $20 per hour Overnight Shift – 12 hour@ $22 per hour
Friday-Sunday Day Shift – 12 hour @ $23 per hour Overnight Shift – 12 hour @ $25 per hour
QUALIFICATION REQUIREMENTS:
EDUCATION, EXPERIENCE and CERTIFICATIONS:
- High School Diploma or Equivalency/ GED
- Completion of all required WNC Direct Care and state-mandated training. (provided by WNCDC)
- First Aid/CPR
- Bloodborne Pathogen & Med Administration
- NCI Certification
- Good verbal communication skills
- Ability to work as part of a team
- Valid driver’s license, current automobile liability insurance, safe and reliable transportation,
PHYSICAL/MENTAL DEMANDS:
- Physical requirements include:
- ability to sit, stand, squat, bend, pivot, and walk
- lift objects weighing over 25 pounds
- Physically, mentally, and emotionally fit to perform the following actions:
- rapidlypursue service recipient if he/she tries to elope
- use Crisis Prevention techniques (NCI Training Provided) for potential behaviors
Job Types: Full-time, Contract
Pay: $20.00 - $25.00 per hour
