We are seeking a dedicated and organized Memory Care Coordinator to join our healthcare team. Responsibilities
- Develop and implement individualized care plans in collaboration with healthcare providers and patients.
- Utilize electronic health record systems such as Point Click Care to document patient information accurately.
- Ensure compliance with HIPAA regulations while handling sensitive patient information.
- Coordinate appointments, referrals, and follow-up care for patients.
- Maintain clear communication with patients regarding their care plans, treatment options, and any necessary follow-up actions.
- Monitor patient progress and make adjustments to care plans as needed to ensure optimal health outcomes.
Qualifications
- Have a minimum of 1 year working experience with Alzheimer's/Dementia.
- Previous experience in a healthcare setting, preferably as a Care Coordinator or similar role.
- Familiarity with care plans and electronic health records (Point Click Care).
- Knowledge of HIPAA regulations and commitment to maintaining confidentiality of patient information.
- Strong organizational skills with the ability to manage multiple tasks simultaneously.
- Excellent communication skills, both verbal and written, to effectively interact with patients and healthcare professionals.
- Ability to work collaboratively within a team-oriented environment while demonstrating strong problem-solving skills.
- Successful candidate will have a deep understanding of memory impairments and best practices in caring for individuals with such conditions.
