Position Overview
We are seeking a skilled and compassionate RN Field Nurse to join our team in the Columbus, OH area. As a field nurse, you will provide direct patient care in home settings, ensuring quality treatment, patient safety, and positive health outcomes.
Key Responsibilities
- Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans
- Perform comprehensive assessments, wound care, IV therapy, PEG/ostomy/drain management, and medication administration
- Monitor patient progress, document care, and communicate effectively with physicians, caregivers, and interdisciplinary team members
- Educate patients and families on treatment plans, medications, and preventive health strategies
- Ensure compliance with state, federal, and agency policies and procedures
- Maintain accurate and timely charting in electronic medical records
- Current Ohio RN license in good standing
- Minimum 1 year of nursing experience (Home Health experience preferred, but not required)
- Strong clinical skills in wound care, infusion therapy, and chronic condition management
- Excellent communication, organizational, and time-management skills
- Ability to work independently and as part of a multidisciplinary team
- Valid driver’s license and reliable transportation (travel within Franklin, Licking, Fairfield, and surrounding counties)
- Experience with Home Health or Hospice
- Familiarity with electronic medical charting systems
- Ability to work with multi-cultural patient populations, including non-English speaking families
