Heart Transplant Coordinator

We are seeking an experienced and compassionate Heart Transplant Coordinator for a temporary assignment. The Heart Transplant Coordinator will work closely with physicians and other healthcare providers to support the planning, evaluation, coordination, and facilitation of care for patients and families undergoing the organ transplantation process.

The Heart Transplant Coordinator will serve as an important resource throughout the transplant process, helping ensure that patients and families receive appropriate support and coordination of care.

Responsibilities

• Coordinate and facilitate the care process for patients undergoing heart transplantation.
• Collaborate with physicians and other healthcare providers involved in the transplant process.
• Assist with the planning and evaluation of patient care.
• Provide support and coordination for patients and their families throughout the transplantation process.
• Serve as an advocate for patients and families.
• Collaborate with healthcare professionals and other members of the care team.
• Apply clinical knowledge and professional expertise to support quality patient care.
• Serve as an educator and resource for patients, families, and healthcare professionals as appropriate.
• Demonstrate sound clinical judgment and knowledge related to organ transplantation.
• Maintain professional standards and clinical expertise while performing Heart Transplant Coordinator responsibilities.

Qualifications

• Clinical healthcare experience with knowledge of the organ transplantation process.
• Strong communication, coordination, and interpersonal skills.
• Ability to collaborate effectively with physicians and multidisciplinary healthcare teams.
• Ability to provide patient and family education and support.
• Strong organizational skills and attention to detail.
• Ability to advocate for patients and families while maintaining professional standards.
• Demonstrated clinical knowledge and sound judgment.
