Wingspan Life Resources is seeking a House Coordinator (HC) for our Youth Specialized Services Crisis home in Bloomington, MN. The clients at this home have developmental disabilities and mental illness and range in ages from 12 - 17. In addition to the responsibilities of a DSP, the HC supervises a team of employees. The HC is responsible for all on-site operational aspects of the home in accordance with the mission, policies, and procedures of Wingspan, and complies with state, county and federal licensing requirements.
The House Coordinator is responsible is responsible to protect and promote the health, safety and well-being of each client, assuring services are delivered according to Wingspan Life Resources policies and procedures along with county, state, federal and licensing rules and regulations.

SUMARY OF DUTIES:
HC Responsibilities:
- Using principals of person centered planning, develops, implements and manages active treatment plans for the individuals living at the sites.
- Continuously assess the client’s medical and mental health care plans, regularly communicating progress, changes and recommendations to the PD and to staff members responsible to implement the plans.
- Assure staff members are correctly following all client(s) protocols/plans at all times and completing all required documentation.
- Monitor the performance of staff, providing ongoing coaching and feedback, communicating recommendations and observations, both positive and negative, to the PD
- Responsible for assuring the purchasing of gas, groceries, household items and the reordering of medications.
- Assisting DSP’s with initial and continuing training and supervision that enables the employee to perform their duties effectively, efficiently and competently
- Assisting staff compliance with Wingspan Policy and Procedures and DHS service requirements at all times.
- Reviews daily required documentation, including incident reports that occur at the home and to follow the guidelines of reporting incidents to support teams of the clients served.
- Meet regularly with the PD communicating any concerns regarding employees, clients, services, maintenance or daily operations.
- Promotes active treatment of clients in all aspects of life through choice, participation in tasks of daily living, and encouraging independence as appropriate.
- Assists in developing and implementing assigned program and behavior plans.
- Completes daily progress notes and accurate data collection for outcomes and plans.
- Plan and/or facilitate activities that are in the best interest of the client which supports their cultural needs and choices and may represent their sexual orientation, gender, religious beliefs, etc.
- Demonstrates respectful interactions with the clients, visitors, family members, guardians, community members, county representatives, consultants, other employees, etc.
- Re-direct and correct clients sensitively, respectfully and for their benefit
- Initiates cleaning tasks as needed to maintain a clean house, yard, agency vehicle, grass, snow removal.
- Leaves the house and grounds in a neat and tidy condition at the end of the shift
- Follows prescribed accounting methods for managing expenditures for the household and for the client.
- Supervises and, if necessary, assists clients in personal purchases
- When necessary, complete purchases for clients, considering their personal taste, their current budget and upcoming expenses, following up with accurate accounting and documentation of all expenses
- Responsibly completes needed purchases for the house following the timelines and procedures to account for each purchase using company credit card.
- Complete expense and mileage reports appropriately and within timelines set by Wingspan.
Essential Job Requirements:
- 18 years of age or older
- Ability to meet licensing requirements of state and county
- Ability to read, write, and communicate effectively in the English language
- Ability to work effectively within a team and maintain a positive demeanor
- Ability to work independently and without continuous on-site supervision
- Demonstrated leadership skills
- Ability to use computer programs for personal timesheets and client documentation
- Works in a variety of settings inside and outside the site and throughout the community.
- May be subject to emotionally upset and physically aggressive clients.
- May be exposed to infectious illnesses, odors, dust, pets, etc. throughout the day.
- Valid MN Drivers’ License and a driving record that meets Wingspan’s requirements. May use personal vehicle and/or company vehicle during work shift.
- Assures the environment is safe, harassment and discrimination free. Reports immediately any unsafe work practices or hazards and any harassing or discriminating practices observed or suspected.
- Ability to lift and carry up to 50 pounds
- Sits, stands, walks, bends, lifts, pushes, pulls, and moves intermittently during work hours.
- Ability to complete tasks such as shoveling, shopping, mowing, vacuuming, pushing a wheelchair, etc.
- High School Diploma, G.E.D. or equivalent.
- Degree in a related field preferred
- 3 years experience working with individuals with disabilities and mental illness preferred
- Specialized Service sites: 1 year experience working with individuals with high behaviors preferred
- One year experience working with individuals with disabilities and mental illness and/or a degree in Psychology or Social Services
