What are the responsibilities and job description for the Patient Navigator - Continuous Improvement Dept position at Wind River Family and Community Health Care?
Assist patients in navigating the healthcare system, including how to schedule appointments, coordinating referrals, and connecting with appropriate internal and external resources.
Provide education and support to patients and families regarding medical terminology, treatment plans, and follow-up care
Advocate for patients' needs and preferences while respecting cultural values and beliefs.
Identify and help resolve barriers to care, such as transportation, insurance, or communication challenges.
Maintain up-to-date knowledge of healthcare services, insurance processes, and local support programs.
Document patient interactions and maintain confidential records in compliance with HIPAA and organizational policies.
Collaborate with healthcare providers, social workers, and case managers to ensure continuity of care.
Attend all weekly POD Meetings.
Attend all Clinical Staff Meetings.
Attend all Nursing staff meetings.
Completion of checklists and summary reports and data collection.
Document all contacts and services in narrative format and enter data into the appropriate data base.
Obtain all required signatures for service delivery.
Participate in outreach events, health fairs, and patient education activities.
Monitor patient progress and follow up to ensure care plans are completed.
Other duties as assigned to maintain and enhance the operation of the department.
