What are the responsibilities and job description for the Maternal Child Health Nurse - Clinical Nursing position at Wind River Family and Community Health Care?
Provides assessment and teaching and foster independence with breastfeeding and monitor infant growth and development. MCH Nurse develops evaluates and revises patient care goals in conjunction with patient and family.
Communicates with medical providers, family and interdisciplinary care providers to clarify care and increase awareness of significant changes in condition in a timely manner.
Maintains effective working relationships with patients/families, referral sources, physicians, community resources and insurance representatives.
Builds strong working relationships internally with providers, nurses, First Impressions, and Benefit Coordinators to ensure patient has access to best possible healthcare.
Carries out the Agency's mission, philosophy, goals, and objectives within guidelines of Agency policy and position function.
Assesses and evaluates the health care needs of patients and families with consideration regarding physiological, psychological, social, and environmental factors.
Develops a care plan based on patient needs, physician orders, nursing assessment parameters and identified goals that are respectful of patient, family, community, and agency resources.
Implements the care plan and revises it whenever necessary by regularly assessing, observing, and evaluating the patient's condition, needs and response to care, and makes appropriate nursing judgments and decisions for care plan revision.
Initiates and applies appropriate preventative, therapeutic and rehabilitative nursing procedures, and techniques.
Administers medications and treatments as prescribed by the physician and performs nursing procedures and techniques.
Teaches the patient and family/caregivers self-care techniques whenever appropriate and provides instruction regarding medication, diet, safety, and treatment modalities in accordance with the plan of care.
Completes, maintains, and submits all required documentation that is timely, accurate and relevant.
Performs complete prenatal assessment, complete postpartum assessment and completes infant assessment utilizing EHR documentation.
Provides case coordination, health teaching, supportive counseling and advocacy for the patient and family.
As needed connects patient with other departments within WRFCHC to ensure patient has access to all possible programs and resources.
Other duties as assigned to maintain and enhance the operation of the department.
