What are the responsibilities and job description for the Case Manager - Sacred Child position at Wind River Family and Community Health Care?
Provide daily administrative support to the Sacred Child Administrative Director.
Maintain accurate, organized, and confidential records for youth, staff, and program operations.
Prepare reports, summaries, and other documentation for program leadership and compliance purposes.
Assist in developing, updating, and implementing program policies and procedures.
Coordinate program communications, staff schedules, and meeting logistics.
Conduct intake assessments and create individualized service/care plans for youth.
Monitor and document youth progress, behavior, and outcomes.
Collaborate with medical providers, schools, counselors, social services, and other community partners to meet youth needs.
Advocate for children's safety, well-being, and developmental goals within the program and community.
Provide safe and reliable transportation for children to doctor's appointments, family visitations, court hearings, and other approved activities.
Participate in team meetings, case reviews, and multidisciplinary planning sessions.
Provide crisis intervention and support when needed, ensuring child safety at all times.
Partner with child care and residential staff to ensure daily needs of youth are consistently met.
