Resident Living Social Worker

Willow Valley Communities Independent Living is seeking a Resident Living Social Worker to join their team! The Resident Living Social Worker is responsible for coordinating services for Residents in Residential Living to ensure that their psychosocial needs are met utilizing an interdisciplinary approach. Responsibilities: Coordinates with Manager of Resident Services on all Resident concerns. Works with other departments in familiarizing new Residents to the community. Participates in weekly meetings with Supervisor to discuss any concerns and Resident interactions. Mediates using problem solving when there are Resident to Resident conflicts, offering internal and external resources as necessary. When Residents are hospitalized, consult with WVC Admission Coordinator on discharge planning and work with WVC Chaplains to meet psychosocial needs of those hospitalized Residents and their spouses. Collaborate with Residents, their spouses/families, Resident Nursing, Administration, Physicians and Continuing Care Assessment Team to make appropriate transitions through the continuum of care. Recognizes and explores the transition of Residents who are faced with health and safety concerns. Coordinates support services for Residents and/or their spouses through difficult, life changing transitions, i.e., loss of driver#s licenses, finding alternative transportation, chronic illness and decline in mobility or activities of daily living, arranging home care services, temporary or long term move to Supportive Living, hospitalization, death of spouse. Knowledgeable of all Resident Agreements, Resident Handbook and contractual requirements (ie. insurance requirements). Directs communication regarding Resident and/or Responsible Party at time of vacating residence whether due to permanent transfer to Supportive Living or at the time of a Resident#s death.
