- Lead and oversee the hospital’s care management operations, ensuring coordinated patient care across departments while working closely with physicians, care managers, payers, patients, and families to support quality outcomes and efficient use of resources.
- Guide utilization management and discharge planning efforts, using knowledge of regulatory requirements and payer guidelines to support appropriate length of stay and smooth patient transitions through the care continuum.
- Manage and mentor a team of case managers, providing leadership, oversight, and performance guidance while ensuring compliance with healthcare regulations and leveraging data to monitor performance and improve care coordination.
- BSN Required. MSN or MPA Preferred
- 5 years experience in an acute care setting including 2 years experience in hospital utilization review and or managed care
- 1 years of management experience
- Case Management certification preferred
- Current New York state RN license required
