In need of Hospice Chaplain in Victoria, Goliad, Beeville and Refugio, TX.
QUALIFICATIONS
- Ordained and endorsed minister, priest, rabbi, member of a religious order, or a layperson with certified pastoral care training.
- Personal and/or professional experience supporting individuals and families facing death and dying.
- Comfortable and knowledgeable working with diverse spiritual and religious beliefs and practices.
- Working knowledge of community-based spiritual resources.
- Strong interpersonal, communication, and listening skills.
- Annual TB screening required.
- Ability to complete point-of-service electronic or paper charting; basic computer/tablet proficiency.
RESPONSIBILITIES
Spiritual Support & Presence
- Provide a compassionate spiritual presence aligned with the foundational values of hospice care.
- Support patients and families in accessing spiritual resources of their choosing.
- Offer direct spiritual counsel, prayer, sacraments, and rituals when requested.
- Interface with the patient's clergy, congregation, or spiritual community upon request.
- Be available to be present at the time of death if requested.
- Provide spiritual literature or resources that may support patients and families throughout their journey.
Interdisciplinary Team Collaboration
- Participate actively in the patient/family care plan as a member of the interdisciplinary team.
- Document spiritual assessments, needs, resources, and progress notes in the patient record following each visit.
- Complete Bereavement Risk Assessments in accordance with organizational guidelines.
- Attend IDG meetings every other Thursday, offering a spiritual opening as needed.
- Raise potential spiritual concerns during patient care conferences and report interventions.
- Provide spiritual and emotional support to team members, especially during times of grief or patient loss.
- Attend scheduled quality improvement meetings, staff meetings, and required in-services.
Patient & Family Support
- Encourage patients and families to consider advance arrangements such as funerals, wills, obituary details, and interment plans.
- Officiate funeral services when requested.
- Ensure spiritual needs are accurately documented during intake/assessment and throughout the plan of care.
