Case Manager, Homeless Prevention Services

The Homeless Prevention Case Manager provides time-limited case management for families that are most at risk of entering shelter system to help them maintain or reestablish housing stability in the community. Using strong interpersonal skills and discernment, the Case Manager is responsible for providing for the safety concerns of the families, homelessness risk assessment and mediation services. They also utilize their knowledge of community resources to provide families with connection to legal services, case coordination, and other supportive services in the community as need be.
Duties and Responsibilities:
· Assesses families for homelessness risk and provides diversion, family mediation, housing counseling and search, financial management assistance, and direct financial assistance to ensure housing stability.
· Provides appropriate referrals, including linkages to homeless services, employment services, legal services, and financial management services.
· Meets weekly and as needed with families on caseload to develop case plans, review progress, and to do follow-up on tasks and services.
· Provides housing stabilization services to ensure that families maintain housing, including collaborating with other service providers, and following through on referrals for other needed services.
· Networks and coordinates with community resources for housing, legal services, employment, substance abuse treatment, mental health treatment, training, etc., and makes referrals as appropriate.
Minimum Qualifications:
· Bachelor’s degree in social work or related field required.
· Minimum one-year work experience in social work or related field.
· Experience working with families, homelessness, and/or substance abuse strongly preferred.
· Knowledge of community resources or the ability to become knowledgeable required.
· Strong interpersonal, mediation, and conflict-resolution skills required.
· Strong organizational skills required.
