Position Overview
We are seeking a Home Health Registered Nurse (RN) to provide skilled nursing services to adult and pediatric patients in their homes. This position primarily serves Spanish-speaking patients, therefore fluency in both English and Spanish is required to ensure safe patient care, effective education, and accurate clinical documentation.
Key Responsibilities
- Perform skilled nursing visits in the home, including:
- Start-of-Care (SOC), resumption-of-care (ROC), and recertification visits
- Routine skilled nursing visits and patient assessments
- Complete comprehensive assessments and develop nursing care plans in collaboration with the physician and interdisciplinary team
- Provide skilled nursing interventions such as:
- Medication management and patient education
- Wound care and dressing changes
- Disease management and clinical monitoring
- Provide patient and caregiver education in Spanish for Spanish-speaking patients
- Communicate effectively with patients, families, physicians, therapists, and agency staff
- Accurately document all visits, assessments, care plans, and communications in the EMR in English
- Participate in care coordination, case conferences, and discharge planning
- Identify changes in patient condition and initiate appropriate clinical escalation
- Maintain compliance with all state, federal, payer, and agency requirements
Qualifications
- Active Florida Registered Nurse (RN) license in good standing
- Minimum 1 year of RN experience (home health preferred)
- Fluent in both English and Spanish (required)
- Strong assessment, clinical judgment, and care coordination skills
- Ability to work independently in the field
- EMR documentation proficiency
- Reliable transportation and valid driver’s license
- Current CPR/BLS certification
- Ability to meet all background screening and compliance requirements
