Home Health Registered Nurse

Position Overview

We are seeking a Home Health Registered Nurse (RN) to provide skilled nursing services to adult and pediatric patients in their homes.

Key Responsibilities

• Perform skilled nursing visits in the home, including:
• Start-of-Care (SOC), resumption-of-care (ROC), and recertification visits
• Routine skilled nursing visits and patient assessments
• Complete comprehensive assessments and develop nursing care plans in collaboration with the physician and interdisciplinary team
• Provide skilled nursing interventions such as:
• Medication management and patient education
• Wound care and dressing changes
• Disease management and clinical monitoring
• Provide patient and caregiver education
• Communicate effectively with patients, families, physicians, therapists, and agency staff
• Accurately document all visits, assessments, care plans, and communications in the EMR
• Participate in care coordination, case conferences, and discharge planning
• Identify changes in patient condition and initiate appropriate clinical escalation
• Maintain compliance with all state, federal, payer, and agency requirements

Qualifications

• Active Florida Registered Nurse (RN) license in good standing
• Minimum 3 year of RN experience (home health preferred)
• Strong assessment, clinical judgment, and care coordination skills
• Ability to work independently in the field
• EMR documentation proficiency
• Reliable transportation and valid driver’s license
• Current CPR/BLS certification
• Ability to meet all background screening and compliance requirements
• Fluent in Spanish (is a plus but not required )

Pay: $1,000.00 - $1,500.00 per week
