Duties
- Conduct thorough MDS assessments for residents or patients, ensuring all data accurately reflects their clinical status and care needs
- Collaborate with interdisciplinary teams including nursing, therapy, social work, and medical staff to gather comprehensive information for documentation
- Review medical records, clinical documentation, and coding to verify accuracy and completeness in accordance with CMS (Centers for Medicare & Medicaid Services) standards
- Maintain up-to-date knowledge of regulations related to MDS, RAI (Resident Assessment Instrument), HIPAA (Health Insurance Portability and Accountability Act), and other compliance requirements
- Utilize EMR (Electronic Medical Record) systems such as Epic or Cerner to input data efficiently and securely manage patient information
- Assist with Medicare reimbursement processes by ensuring proper coding using ICD-10 (International Classification of Diseases), CPT (Current Procedural Terminology), ICD-9, DRG (Diagnosis-Related Group), and other relevant coding systems
- Support discharge planning, case management, and utilization review activities by providing accurate documentation aligned with clinical standards
- Participate in quality assurance initiatives including clinical documentation improvement programs and compliance audits
- Stay current on healthcare trends related to acute care, inpatient/outpatient services, ICU (Intensive Care Unit), PICU (Pediatric ICU), trauma centers, hospice care, and home health environments
Qualifications
- Proven experience with MDS coordination within skilled nursing facilities or long-term care settings in Indiana
- Strong knowledge of EMR systems
- Expertise in ICD-10/ICD-9/ICD coding, CPT coding, DRG assignment, and medical documentation review
- Excellent understanding of HIPAA regulations to ensure confidentiality and secure handling of medical records
- Ability to interpret complex medical terminology accurately while maintaining attention to detail in documentation review
- Strong organizational skills with the ability to manage multiple priorities in fast-paced environments such as outpatient clinics or inpatient units
- Nursing background or clinical licensing is advantageous; familiarity with discharge planning, case management, or health information management is a plus
