We are seeking an ID/DD Manager for our ID/DD Case Management Department
Responsibilities & Duties:
- Providing leadership to staff responsible for care coordination and case
management and for establishing policy and procedure that ensures timely
access to high quality care
- Provides educational trainings to case management team members on
Regulations, Community Supports, Community Transition Planning,
Employment, and other needs as dictated by DBHDS relative to ID/DD Services
- Facilitates access to quality supports for consumers in community and
home-based programs
- Works collaboratively with other clinical and administrative staff in
planning, developing, implementing and evaluating programs to
support and enhance client care across the continuum
- Provides hands-on support with cases as needed in the community
- Facilitates resolutions to concerns of consumers, parents/guardians and staff
- Develops policy and procedure to promote timely and appropriate person-
centered supports
Qualifications and Skills:
- BA/BS degree in Human Service Field
- QIDP
- Three (3) years of experience supervising and evaluating subordinate staff (Preferred)
- Experience supporting individuals with ID/DD.
- Valid Driver’s License and acceptable DMV Record
