We are seeking a (Social Worker) for our (DD Residential Services).
Responsibilities & Duties for (Social Worker):
MAJOR DUTIES AND RESPONSIBILITIES
- Assist in planning, developing, organizing, implementing, and evaluating the social work plans of care for the ICF/ID/DD population.
- Meet with administration, medical, and nursing staff, and other related departments in planning social work plans of care for residential consumers.
- Keep a knowledge of current Federal and State regulations, as well as, professional standards, and make recommendations accordingly for changes to policies and procedures.
- Attend ID/DD Team meetings.
- Participate in discharge planning and consumer assessment.
- Interview consumers/families as necessary.
- Perform administrative job duties, such as completing necessary forms/reports and submitting such to the Program Supervisor.
- Involve the consumer/family in planning social work plans of care as necessary.
- Advise the consumer/family of the consumer’s Human Rights and property rights.
- Consult with program staff and community agencies to identify consumer needs and/or problems via the development of social work plans of care.
- Maintain confidentiality of all patient care information to assure protection of consumer rights.
- Work with the program consultants to identify, implement and recommend changes to the consumer’s social work plans of care.
- Provide problem-solving solutions, financial assistance, discharge planning, and referrals to other community agencies when specialized assistance is required.
Qualifications and Skills for (Social Worker):
- BSW from an accredited School of Social Work; MSW preferred.
- One (1) year related work experience; two (2) years preferred.
- Valid Driver’s License and acceptable DMV record at the time of employment and continuously thereafter. Termination of employment may result if driving record becomes unsatisfactory
