The Telephonic Care Home Coach is a vital member of the member-centric TC Team, introducing members to Western Reserve Area Agency on Aging and supporting their goals of well-being and independence. You will provide member-centered coordination of services and act as a bridge between members and managed care organizations.

The Telephonic Care Home Coach is a vital member of the member-centric TC Team, introducing members to Western Reserve Area Agency on Aging and supporting their goals of well-being and independence. You will provide member-centered coordination of services and act as a bridge between members and managed care organizations.
Key Responsibilities:
- Introduce program elements to participants and explain medications, scope of care, transition events, and agency resources
- Conduct prescreening assessments, chart histories, and initial intake evaluations
- Coach participants on personal goals, medication compliance, and self-advocacy
- Educate and support caregivers about agency resources
- Advocate for participants and secure appropriate services
- Apply clinical knowledge and critical thinking to perform program activities
- Report abuse, neglect, or exploitation of older adults as required by Ohio law
- Participate in training, meetings, and continuing education opportunities
- Collaborate with external agencies and internal departments
- Exercise discretion and independent judgment to support agency operations
- Licensure : RN, LPN, or LSW
- Minimum one year of clinical experience post-licensure
- Excellent customer service and communication skills
- Basic computer skills, including familiarity with Microsoft Office applications
- Ability to travel to community and participant locations within service areas
- Must have a car available during working hours with current insurance
