About Us:

Western Reserve Area Agency on Aging (WRAAA) is a private non-profit corporation, organized and designated by the State of Ohio to be the planning, coordinating and administrative agency for federal and state aging programs in Cuyahoga, Geauga, Lake, Lorain, and Medina Counties. It is one of twelve Area Agencies on Aging (AAAs) in the state organized with local service provider organizations and the Ohio Department of Aging (ODA) to form the state's public aging services network. The network collaborates to create opportunities for older Ohioans to receive needed home and community services and support; and to age successfully in their own homes and communities.

Duration:  This is an non-paid internship for the duration of the schools need.

Job Overview:

This role is integral to promoting the well-being and independence of members, ensuring seamless transitions of care, and preventing re-hospitalizations. The intern will serve as a key interface between members, healthcare providers, and various care programs, fostering high-quality support and collaboration.

Key Responsibilities:

• Act as the primary contact for members, explaining program benefits, care scope, and resources.
• Engage with members to enroll them in care programs, monitor their involvement, and facilitate their participation in services.
• Conduct assessments via home visits or phone calls to determine the needs and progress of members.
• Work alongside a multidisciplinary team to ensure effective care coordination and liaise with service providers to meet program requirements; report any concerning incidents, like abuse or neglect.
• Educate program participants and caregivers on medication adherence, personal goals, and available resources.
• Maintain detailed records of member progress and interactions, adhering to agency policies and quality improvement initiatives.
• Connect clients with community resources and advocate on their behalf, participating in outreach activities to foster relationships with service providers.
• Engage in training and supervision to maintain proficiency in care coordination strategies and abide by professional ethics.
• Travel within specific regions to support program goals and meet participant needs.
• Uphold the mission, vision, and strategic plan of the organization, utilizing clinical judgment to perform duties aligning with professional standards.

Qualifications:

• Licensed Social Worker (LSW) in the state of Ohio is required.
• This opportunity is available exclusively to students enrolled in a Master’s degree program in social work.
• Strong organizational and communication skills, with an ability to prioritize tasks and handle multiple responsibilities in a fast-paced environment.
• Proficiency in Microsoft Office (Word, Excel, TEAMS) and Health Information Exchange (HIE) systems.
• Compassionate, patient-focused approach to service delivery.
