About Us: Western Reserve Area Agency on Aging (WRAAA) is a private non-profit corporation, organized and designated by the State of Ohio to be the planning, coordinating and administrative agency for federal and state aging programs in Cuyahoga, Geauga, Lake, Lorain, and Medina Counties. It is one of twelve Area Agencies on Aging (AAAs) in the state organized with local service provider organizations and the Ohio Department of Aging (ODA) to form the state's public aging services network. The network collaborates to create opportunities for older Ohioans to receive needed home and community services and support; and to age successfully in their own homes and communities.
Duration: This is an non-paid internship for the duration of the schools need.
Overview: This internship is designed for students passionate about addressing social determinants of health (SDoH) by directly engaging with clients through both telephonic and in-person assessments. As a Social Work intern, you will develop essential skills in identifying client needs, linking them to community resources, and supporting holistic health and independence.

- Conduct comprehensive telephonic and in-person assessments with clients to identify SDoH factors affecting their well-being.
- Develop a deep understanding of clients' social, economic, and environmental circumstances through effective questioning and active listening.
- Collaborate with interdisciplinary teams to create individualized care plans that address identified SDoH barriers.
- Facilitate referrals and coordinate access to community resources such as housing assistance, transportation services, food programs, and mental health support.
- Maintain accurate documentation of assessments, interventions, and outcomes in line with regulatory requirements and organizational standards.
- Provide ongoing support and follow-up with clients to ensure needs are met and barriers to accessing recommended resources or services are addressed.
- Stay informed about changes in social services, community resources, and healthcare policies to provide up-to-date information and support to clients.
- Communicate effectively with clients, healthcare providers, and community organizations to foster collaborative relationships.
- Participate in team meetings and training sessions to enhance skills in care coordination strategies and client engagement.
- Enrollment in a Bachelor’s or Master’s program in Social Work.
- Strong organizational skills and the ability to handle multiple tasks efficiently.
- Exceptional communication and interpersonal skills for interacting with clients, families, and community partners.
- Compassionate approach with a focus on promoting client independence and well-being.
- Proficiency in conducting assessments and documenting findings accurately.
- Basic proficiency in Microsoft Office (Word, Excel, Teams) and digital communication tools.
- Commitment to understanding and resolving issues related to social determinants of health and improving health equity.
