Status: Full-Time 35 hours/week

We are looking for a compassionate, resourceful, and highly skilled Information & Referral/Assistance (I&R/A) Specialist, to join our team. The Information & Referral/Assistance (I&R/A) Specialist plays a central role in advancing WCAAA’s mission to connect older adults, individuals with disabilities, and caregivers in Northwest Connecticut to needed services, supports, and educational resources. This position provides timely, accurate, and person-centered information and assistance while also conducting public education to raise awareness of aging and disability services.

The Specialist ensures compliance with Title III-B federal requirements and state reporting standards, working closely with other WCAAA staff and community partners to deliver high-quality, accessible services.

What You’ll Do

Information & Assistance

  • Provide person-centered information and assistance to older adults, individuals with disabilities, caregivers, and professionals.
  • Conduct light screening of client needs and connect individuals with appropriate resources, programs, and supports (e.g., housing, transportation, financial assistance, Medicare/Medicaid, home- and community-based services).
  • Make direct referrals or provide detailed contact information, ensuring smooth navigation of local, state, and federal resources.
  • Document all client interactions using Microsoft Forms and maintain accurate records in accordance with Bureau on Aging requirements.
  • Conduct follow-up with clients to ensure needs were addressed and services accessed.
  • Complete CHOICES & AIRS certification within 4-6 months of hire.

Public Education & Outreach

  • Plan, coordinate, and deliver presentations, workshops, and educational events at senior centers, housing sites, libraries, hospitals, and community centers.
  • Collaborate with WCAAA communications staff to disseminate information through newsletters, radio shows, and other media outlets.
  • Track outreach events, including audience size and type of distribution, and report quarterly to the Bureau on Aging.

Coordination & Collaboration

  • Partner with internal programs (ADRC, CHOICES, Caregiver Support, Nutrition, LiveWell, Respite, Housing, SMP, etc.) to ensure integrated service delivery.
  • Support cross-waiver outreach efforts, such as health fairs and community events.
  • Coordinate referrals with state and community partners, including 2-1-1, hospitals, and municipal agents for the elderly.

Data Collection, Reporting & Quality Improvement

  • Enter service delivery data into Bureau-compliant reporting workbooks for quarterly submission.
  • Review client satisfaction survey results and recommend program improvements based on identified needs or service gaps.
  • Participate in required training and professional development to maintain compliance with Title III-B standards.

What We’re Looking For

  • Bilingual in Spanish strongly preferred.
  • Knowledge of aging services, long-term options, and caregiver issues.
  • BA/BS degree in human services or related field preferred; Associate’s degree with 5–7 years of relevant experience considered.
  • Direct experience in human services (especially working with seniors) preferred.
  • Strong interviewing, counseling, and communication skills.
  • Proficiency with Microsoft Office applications.
  • Valid CT driver’s license and reliable transportation.
  • Ability to lift/carry 5–10 lbs. and work in diverse environments.
