Brief Description

Position Title: Crisis Care Coordinator

Reports To: Program Director

FLSA Status: Non-Exempt

Summary

As a member of the Crisis Division Care Coordination team, the Care Coordinator provides direct support to LEAD and QRT clients who are experiencing substance use, mental health challenges, or crisis-related needs. This position acts as a service navigator, providing assessment, individualized service planning, linkage/referral, monitoring, follow-up, transportation, crisis response support, and documentation. The Care Coordinator works closely with law enforcement, first responders, and community partners to ensure clients receive person-centered, harm-reduction–focused care.

Essential Duties and Responsibilities include the following. Other duties may be assigned to meet program needs.

  • Acts as Westbrook’s primary point of contact for LEAD and QRT clients.
  • Responds to referrals from law enforcement, EMS, and partner agencies to engage individuals immediately following overdose, crisis encounters, or low-level law violations.
  • Provides person- and family-centered care to individuals with substance use and/or mental health needs.
  • Initiates, supports, and sustains engagement in treatment, harm reduction, and recovery services.
  • Serve as a guide/mentor to help clients overcome personal and environmental barriers that jeopardize stabilization and recovery.
  • Links and refers individuals to community resources, including harm reduction services, detoxification, treatment, housing supports, family services, recovery coaching, and local or online support groups.
  • Assures transportation needs are met, including transporting clients to appointments when necessary.
  • Advocates for clients in accessing physical, behavioral, legal, and social services.
  • Provides supportive counseling within the scope of training and competencies.
  • Offers hope and encouragement to prevent relapse, reduce criminal-legal involvement, increase safety, and support long-term recovery.
  • Provides Targeted Case Management as applicable for eligible clients.
  • Delivers care coordination in diverse settings including the community, client homes, Westbrook offices, and field-based crisis response with LEAD/QRT partners.
  • Participates in multidisciplinary team meetings with law enforcement, EMS, outreach staff, harm reduction organizations, and treatment providers.
  • Collects and submits service data to required agencies, including program reporting for LEAD and QRT initiatives.
  • Complies with all federal, state, and organizational policies including confidentiality requirements under HIPAA and 42 CFR Part 2.
  • Demonstrates knowledge of and supports Westbrook Health Services’ mission, vision, values, policies, and ethical standards.

Qualification Requirements

  • Must be able to communicate clearly and accurately with a variety of individuals in order to convey and receive information effectively.
  • Must work well in a team environment.
  • Work with staff, consumers and others in a professional, cooperative, ethical, respectful and effective manner.
  • Ability to work with a diverse group of people
  • Must have valid driver’s license and clean driving record

Education and/or Experience: Bachelors degree in Human Services discipline (Psychology, Criminal Justice, Regents with health specialization, Recreational therapy, Political Science, Nursing, Sociology, Social Work, Counseling, Teacher education, or Liberal Arts) or a Bachelor’s degree in another discipline with a minimum of 12 credit hours in the human services disciplines listed previously. Prior computer/PC experience using computerized Medical Records and scheduling systems is preferred, but not required. Incumbent must also possess basic math skills. Must pass Criminal Investigation Background and DHHR background check as well as post offer drug screen.

The employee is often required to sit and use his or her hands and fingers, to handle or feel. The employee is occasionally required to stand, walk, reach with arms and hands, climb or balance, and stoop, kneel, or crouch. Vision abilities required for this job include close vision. Must be able to travel within catchment area. Work is typically performed in an office environment.
