Brief Description
Youth early diversion behavioral health partnership with the state of West Virginia has the goal of helping youth and young adults up to the age of 25 with mental illness or cooccurring disorders. The Youth Early Diversion care coordinator will work directly with families and the community to identify opportunities to divert youth and young adults to improve referrals, engagement, and evidence-based service delivery for the populations of focus
Essential Duties And Responsibilities
Education and/or Experience:
Bachelors degree preferred, prior computer/PC experience using computerized Medical Records and scheduling systems is preferred, but not required. Incumbent must also possess basic math skills.
Must pass Criminal Investigation Background and DHHR background check as well as post offer drug screen.

Essential Duties And Responsibilities
- Coordinate between all local county agencies to help with the treatment of youth and young adults who are at risk of entering the juvenile justice and criminal systems.
- Work directly with the regional youth service center coordinator in Region 3 to coordinate care for the population of focus.
- Increase referrals to regional youth service center services for the populations of focus prior to arrest, booking, or out-of-home placement.
- Provide an array of treatment and recovery services, including clinical service, for the populations of focus that complement or enhance existing regional youth service center.
- Ensuring all regional youth service center staff are trained appropriately to ensure proper screening, treatment, and participation in peers reviews as necessary.
- Identify local opportunities for multiple pathways to immediate treatment and recovery.
- Promote or facilitate the health insurance enrollment process for the eligible uninsured or underinsured.
- Participate in any and all required trainings.
- Become trained to facilitate trainings locally to other mental health professionals, stakeholders, and community members.
- Provide outreach to and engagement of underserved populations.
- Maintain and provide monthly reporting and submit by monthly deadline.
- Conduct national outcome measures (NOMS).
- Comply with federal, state and company policies, procedures and regulations.
- Maintains confidentiality of all consumer information and comply with HIPAA regulations
- Demonstrates knowledge of and supports Westbrook Health Services mission, vision, values, policies and procedures, operating instructions, confidentiality standards and code of ethical behavior.

- Must be able to independently travel to community worksites and required trainings. Travel is primarily during the work day versus overnight.
- Must be able to set up/pack up supplies and equipment necessary to provide appropriate programming
- Ability to work a non-traditional work schedule, including some evenings and weekends
- Must have valid driver’s license and Automobile Insurance
