Nature of Work: Position responsible for providing direct and supportive care to clients within the Assertive Community Treatment (ACT) program.
ACT is a community-based mobile mental health treatment, rehabilitation and support team serving adults with serious mental health and co-occurring substance use issues. Individuals served by the ACT team are typically those for whom traditional mental health services have been ineffective or inadequate. Work hours require flexibility with day, evening, weekend and holiday coverage to address specific client needs.

Responsibilities
- Serves as a member of the ACT team attending all designated meetings. Provides direct care/assistance to ACT consumers to include: care coordination, crisis de-escalation, transportation and assistance with activities of daily living. Provides supportive counseling services individually and in a group format within the scope of training and competency. Facilitates social and recreational activities. Administers medication with completion of appropriate training and certification. Documents all service activities including collateral contacts in a complete, accurate and timely manner according to WVDHHR licensing regulations and Westbrook policy. Participates in an on-call rotation as a member of a team for evening, weekend and holiday coverage. Monitors clients’ safety needs, including physical, medical, behavioral and psychiatric. Assists with Continuous Quality Improvement Program Maintains a level of attendance consistent with Westbrook Health Services policy. Works with staff, clients and others in a professional, ethical and respectful manner. Works to ensure a safe environment for staff, consumers and associates by following Westbrook’s safety policies.

Qualifications:
- High School Diploma/GED.
- Pass a background check to include WV Cares and the WVDHHR Abuse and Neglect Registry.
- Ability to pass a post offer observed urine drug screen
- Valid Driver’s License and the ability to operate a motor vehicle.
