WSFSSH creates and preserves affordable housing while supporting the development of healthy neighborhoods.JOB SUMMARY The Case Worker is expected to work under the guidance of the Senior Social Worker. They will develop 1:1 relationships with the residents which will enable them to provide a range services to the residents which enable the residents to establish and maintain safe and independent lives in the community.JOB DUTIES & RESPONSIBILITIESResponsibilities include but are not limited to:Maintain accurate and organized case files, records, and reports.Write progress notes after each resident encounter.Develop individualized service plans with residents.Draft correspondence, notices, and reminders for residents as needed.Complete data entry of resident progress in internal databases.Assist residents with general inquiries or referrals to services.Accompany residents to appointments or community activities as needed.Help facilitate social and recreational activities for residents.Assist in preparing materials for group sessions or workshops.Carry a small caseload of residents who present as less challenging.Participate in team meetings and trainings.Assist social workers in emergency situations as directed.Report urgent resident concerns promptly to supervising social workers.REQUIRED KNOWLEDGE SKILLS & ABILITIES Good written and oral communication skills. Good organizational ability, time management, and interpersonal skills. Desire or interest in working and supporting an aging population REQUIRED EDUCATION, EXPERIENCE AND CREDENTIALS Required:High school diploma or equivalent; associate or bachelor's degree in social work, Human Services, or related field preferred.
