JOB OPPORTUNITY
The Perinatal Care Coordinator is responsible for supporting pregnant patients who have psychosocial barriers or are geographically isolated with individualized treatment planning, psychosocial education, and patient support. As a member of the OB team and CPSP team, the Perinatal Care Coordinator participates in supporting patients who may need complex care coordination across medical, dental, mental health, hospital, school-based, community-based services, or other care partners. Performs essential duties of the Comprehensive Perinatal Health Worker job role, including scheduling care navigation, health education, nutrition support, and CPSP services from initial assessment to postpartum period. Assists with home visits to program participants. Supports quality management efforts and grant activities as assigned.
Position is a full time, on-site role located in Sebastopol, California and requires some travel to patient homes.
DUTIES
- Case management: Supports patients with complex psychosocial or geographic barriers with the creation and support of a comprehensive care plan, connecting with community resources, self-management education, and helping patients access community resources.
- Care Coordination: Coordinate patient care with other members of the interdisciplinary team, including clinicians, medical assistants, and registered dietitians. Track and follow up with patients regarding appointments, lab results, and referrals.
- Patient assessment and care planning: Conduct initial and ongoing health education, nutrition, and psychosocial assessments with patients. Based on the assessments, develop and update individualized care plans.
- Health education: Provide counseling and educational materials on topics such as prenatal and infant care, nutrition, breastfeeding, family planning, and stress management.
- Documentation: Maintain accurate and timely records of all services provided to patients in the Electronic Health Record (EHR) system, and manage case loads and utilization reporting.
- Patient outreach and support: Serve as a liaison between patients and medical providers, assisting patients with issues like health insurance applications and navigating the healthcare system. Perform outreach to engage patients and encourage continuity of care.
- Visits: Visits may be conducted in the clinic, or patient’s home, or lived environment.
- Program participation: Participate in required trainings and team meetings to stay up to date with CPSP guidelines, health trends, and program goals.
- Operational and Grant Support: May assist with grant reporting, community meetings, and support data collection and activities as assigned by supervisor.
- Care Partner Liaison: Acts as liaison with partnering community service organizations such as Keystone, Child Parent Institute, Public Health Nursing, and Child Protective Services, school partners, and others.
- Meetings: Attends monthly OB Team meetings for case coordination. Attends monthly county CPSP Meetings led by the county PSC and with PHPS as needed.
- Quality Improvement: Assist with clinical data outcome reporting and actively participates in quality improvement projects. Assist in the adoption and implementation of new technologies and activities that improve patient care and clinic efficiencies.
EXPECTATIONS
- High school diploma or GED required. Associate's degree preferred.
- At least 1 year experience in prenatal or perinatal care setting, preferably within a community health center.
- Fluency in a second language, such as Spanish, strongly preferred.
- Strong knowledge of medical terminology and prenatal care services.
- Strong organizational skills and the ability to multitask effectively in a busy environment.
- Excellent interpersonal skills to interact positively with patients and staff alike.
- Proficiency in using office software and electronic health record systems is beneficial.
- Ability to reliably commute to each of our facilities, community meetings and patient homes, as needed
PHYSICAL REQUIREMENTS
- Ability to move freely about the clinic/office and carry light loads (up to 25 lbs.)
- Prolonged standing and /or sitting required.
- Ability to interface with electronic office equipment for extended periods of time.
