What You'll Do
- Conduct in-home client assessments and quality assurance visits.
- Create, update, and maintain comprehensive care plans.
- Build trusting relationships with clients, families, and caregivers.
- Respond professionally to client concerns and help resolve challenging situations with confidence and compassion.
- Support and coach caregivers to ensure the highest standard of care.
- Collaborate with schedulers, healthcare professionals, and office staff to ensure seamless service.
- Maintain detailed documentation and accurate reports.
- Monitor client satisfaction and proactively identify opportunities to improve the client experience.

Qualifications
- Experience in care coordination, case management, nursing, social work, healthcare administration, or senior care preferred.
- Experience creating care plans and maintaining accurate documentation.
- Strong computer skills and ability to learn agency software.
- Excellent written and verbal communication.
- Valid California driver's license, reliable transportation, and willingness to travel throughout Sonoma County.

Travel: Throughout Sonoma, Napa & Marin counties.
