What are the responsibilities and job description for the Hospice Social Worker position at West Coast Hospice Care?
As our growth continues, West Coast Hospice is seeking a Per Diem Social Worker (MSW) in Carlsbad and surrounding areas, to aid in our mission: to provide cost-efficient, quality home care services to the patients entrusted in our care. We are here to exceed the expectations of our patients, clients, and their families by providing excellent clinical care and premier service.

As a Per Diem / Hospice Social Worker you will:
Provide psycho-social assessment and intervention to Hospice patients and their families
Utilizes community resources to assist in resolution of personal and financial problems related to the terminal illness
Counsels and supports patient and family, and act as an advocate to allow for death with dignity and maintenance of optimum family/patient relationships and adjustments
Documentation concise and timely per agency standards
Assists the members of the Hospice team in understanding significant social and emotional factors related to the patient's terminal illness and the family's adjustment to the state of illness
Provides bereavement support as needed; present Presentation of Benefit to patients/families as needed
Follow company policies, procedures and directives
Interact in a positive and constructive manner
Prioritize and multitask
Hospice is both challenging and rewarding This job requires a person who will never forget that home care is a team effort. Other requirements include:
Master's Degree in Social Work from a school accredited by the Council On Social Work Education
Minimum 2-3 years of Hospice Experience
Experience with Homecare Homebase or other EMR not required but a HUGE plus
Ability to communicate effectively, both verbally and in writing
Respecting the patient's privacy and maintaining confidentiality
Current California driver's license and automobile liability insurance
