What You'll Do (Essential Job Functions)
High school diploma or GED
Valid driver's license and reliable transportation

What You'll Do (Essential Job Functions)
- Follow individual care plans and report any concerns to nursing or management.
- Administer medications and treatments with proper documentation, per policy.
- Support daily living needs, including:
- Personal care
- Meal preparation and dietary support
- Household tasks and errands
- Health appointments and community integration
- Building natural supports and social relationships
- Promote health, safety, and a clean home environment.
- Remain calm and effective during challenging situations.
- Foster a positive, respectful team dynamic with coworkers, individuals, and their families.
- Accurately document individual progress and changes.
- Attend all required trainings, meetings, and ongoing professional development.

- Flexible Scheduling: Full-Time & Part-Time - weekend shifts required
