Position Summary:
Wesley Manor Retirement Community is seeking a compassionate, dependable, resident-focused full-time Personal Care Aide (PCA) to join our Memory Care Center team. The PCA provides hands-on, person-centered care and support to residents living with Alzheimer’s disease, dementia, and other memory-related conditions while promoting dignity, comfort, safety, independence, and quality of life. This position assists residents with activities of daily living, provides emotional support and companionship, responds promptly to resident needs, and works closely with nursing leadership and other members of the care team to provide high-quality, compassionate memory care.
Essential Duties and Responsibilities:
- Assist residents with activities of daily living, including bathing, dressing, grooming, toileting, oral care, peri-care, eating, and mobility.
- Support resident dignity, independence, comfort, and safety during all daily care routines.
- Encourage residents to maintain as much independence as safely possible when completing activities of daily living.
- Ensure residents are clean, appropriately dressed, and prepared for meals, activities, appointments, and daily routines.
- Assist residents with scheduled toileting and maintain appropriate toileting documentation in accordance with care assignments and facility procedures.
- Support restorative care by encouraging resident participation and independence with daily activities.
- Promote proper hydration and nutrition by offering fluids between meals and assisting residents with meals and snacks as needed.
- Provide or assist with daily oral care, including brushing teeth and cleaning dentures when residents are unable to perform these tasks independently.
- Assist with resident activities, including one-on-one and group activities, when not providing direct personal care.
- Respond promptly, respectfully, and courteously to resident needs, call lights, and requests for assistance.
- Assist residents with transfers, repositioning, ambulation, toileting, and transportation using proper safety techniques and equipment.
- Take initiative to assist residents as needs arise, regardless of individual assignment, while supporting a team-centered care environment.
- Observe residents and promptly report changes in condition, behavior, skin integrity, pain, safety concerns, or other significant observations to the Shift Supervisor or appropriate nursing personnel.
- Communicate resident, family, or visitor concerns and incidents promptly and professionally to the Shift Supervisor.
- Complete all assigned resident care responsibilities before the end of the shift and communicate the status of resident care before leaving.
- Maintain resident confidentiality and protect all medical and personal information in accordance with HIPAA requirements and Wesley Manor policies.
- Follow all Resident Rights requirements, including dignity, privacy, self-determination, individuality, property rights, and the right to voice complaints or concerns.
- Report known or suspected abuse, neglect, exploitation, safety concerns, or unauthorized disclosure of protected resident information in accordance with facility policy.
- Follow infection control, safety, confidentiality, and organizational policies and procedures.
- Perform other related duties as assigned.
Knowledge, Skills, and Abilities
- High school diploma or equivalent required.
- Previous experience as a Personal Care Aide, caregiver, nursing assistant, or similar direct-care position preferred.
- Previous experience working with residents with Alzheimer’s disease, dementia, or other memory impairments preferred.
- Maintain State Registered/Certified Nursing Assistant status and required continuing education, if applicable to the position.
- Strong communication and interpersonal skills.
- Ability to communicate effectively and compassionately with residents experiencing memory or cognitive impairments.
- Compassionate, patient, and resident-centered approach to care.
- Ability to demonstrate emotional maturity and professionalism when responding to challenging resident behaviors or situations.
- Ability to work effectively as part of a team.
- Self-motivated with the ability to take initiative and respond appropriately to resident needs.
