Key Responsibilities
- Case Management & Advocacy: Conduct initial and ongoing functional assessments to identify residents' needs. Help them apply for programs like SNAP, Medicaid, and utility assistance.
- Service Coordination: Establish and maintain relationships with local healthcare providers, home health agencies, and transportation services.
- Programming & Education: Organize wellness clinics, financial literacy workshops, and social events to combat senior isolation.
- Crisis Intervention: Provide immediate support and referral services during resident emergencies or transitions (e.g., hospital discharges or move-ins).
- Compliance & Documentation: Maintain accurate, confidential resident files in accordance with HUD (or relevant agency) guidelines and privacy laws.
- Mediation: Assist in resolving conflicts between residents or between residents and management to maintain a peaceful living environment
Requirements
- Bachelor’s Degree or Higher in Human Services Field
- Driver’s License
Salary : $22
